    Resident changes, incidents, appointments and risks.

  2. Complete welfare checks

    Confirm every resident seen and recorded.

  3. Support personal care

    Follow individual care plans and dignity standards.

  4. Medication round

    Two-person check for high-risk meds. Sign the MAR chart.

  5. Meals and hydration

    Record intake for residents on monitoring.

  6. Environment and safety walk

    Call bells, fire exits, hoists, spills and equipment.

  7. Update care records

    Notes completed before end of shift.
