Incident Report Checklist

Follow this immediately after any incident so nothing is missed.

Checklist steps

  1. Make the area safe

    Treat injuries and remove ongoing danger first.

  2. Record the facts

    Who, what, where, when. Facts only, no opinions.

  3. Collect evidence

    Photos, CCTV, equipment, witness statements.

  4. Notify internally

    Line manager, safety lead and senior management as required.

  5. Check reporting duties

    Determine if the incident is legally reportable and by when.

  6. Investigate root cause

    Five whys or equivalent. Avoid blaming individuals.

  7. Agree and track corrective actions

    Owner, date and verification of effectiveness.
