Checklist steps

  1. Verify patient identity

    Two identifiers per protocol (e.g. name + DOB).

  2. Confirm and copy insurance

    Front and back of card. Update on file.

  3. Capture consents

    Treatment consent, HIPAA notice, financial responsibility.

  4. Update medical history

    Allergies, meds, conditions, family history.

  5. Take and record vitals

    BP, pulse, respiration, temp, weight, height. Log in EHR.

  6. Triage and route to provider

    Brief chief complaint summary in EHR before provider enters.
