Definition and examples

A RACI matrix is a responsibility assignment chart that maps every project activity against four roles: Responsible (does the work), Accountable (the single owner who signs off), Consulted (provides input), and Informed (kept up to date). Each task must have exactly one Accountable.

Building a feature: PM = Accountable; Engineer = Responsible; Designer = Consulted; CS = Informed.
Hiring: Hiring Manager = Accountable; Recruiter = Responsible; Team lead = Consulted; HR = Informed.

Can two people be Accountable?

No — one Accountable per task. Multiple Accountables means no one is.
